Welcome to Taphys AFC! We’re a Massachusetts state-licensed agency that aims to offer its residents an alternative to facility living or nursing home placement. HOW THE ADMISSION PROCESS WORKS
A referral is made to Taphys AFC (by the potential client or caregiver, the client’s physician, or other agency/person involved with the client’s care)

An assessment of the client and caregiver is completed by a registered nurse and care manager. The home is also assessed for safety to be sure it meets program requirements.

Criminal background checks (CORD) are completed on all household members 18 years of age and older.
A referral is made to Gabriel Care (by the potential client or caregiver, the client’s physician, or other agency/person involved with the client’s care).

Reference checks are verified on the caregiver.

The client’s physician must approve the client for the AFC program.

Caregiver must have a recent physical, tuberculosis screening, and clearance from their physician as it pertains to the responsibilities of the role in question.

Client and caregiver must reside in the same home.
